Collecting personal information is necessary for a Health Professional such as a Naturopath to be able to provide health services. Simone Latta Naturopathy acts in accordance with legislation and regulatory requirements for respecting and maintaining the privacy of your information as a client. This means I comply with the Australian Privacy Principles set within the Privacy Act 1988 (Cth) (Privacy Act).  Please contact me with any questions or concerns around how your personal information is being handled. 
TYPES OF PERSONAL INFORMATION WE MAY COLLECT: 
  • personal details including name, age, gender, date of birth, marital status, current employment status, occupation, level of education, employment history.
  • contact details including billing and delivery address, home address, email address, telephone number and other contact details. 
  • Health & lifestyle information including previous & current health and medical history , allergies, medications or current treatments, immunisations, findings from pathology or other investagatory testing, social history, family history and risk factors 
  • transaction data which may include details about payments to and from you and other details of products and services you have purchased from us.
  • records of our interactions with you.
  • any other personal information that may be required to faciliate provision of services to you.
INFORMATION MAY BE COLLECTED BY VARIOUS MEANS:
  • You book a consultation or purchase a product or service from me. 
  • You contact me to ask a question. 
  • You attend a webinar or event I am hosting or presenting.
  • You interact with me via social media i.e. Instagram, LinkedIn or similar.
  • During appointments where I will ask various questions about your personal circumstances which will be documented securely. 
  • You provide me with a testimonial.
  • A third-party supplies your information to me, for example if you are referred, allied health professional, hospitals, pathology lab, diagnostic imaging services or introduced to me by a mutual acquaintance. 

2. How We Use Your Information

Information is only collected if:
  • I have your full consent to do so, such as when you email me, tick a checkbox or fill an intake form to provide me with information. 
  • to be able to efficiently provide our services that you have requested. 
  • I am legally required to collect it.
  • Information is necessary to preserve life or keep someone safe from harm.
  • you become a client of mine and information is needed for administrative purposes.
We use the information we collect in the following ways:
  •  To Provide Services: To provide, operate, maintain, and improve our Site, products, and services. 
  • To Process Transactions: To process your transactions and manage your orders, including payment processing and delivery of products.
  • To Communicate: To send you information and/or respond to enquiries, including confirmations, invoices, technical notices, updates, security alerts, and support and administrative messages.
  •  Marketing: To send you promotional communications, such as newsletters, special offers, and other information that may be of interest to you. You can opt-out of receiving marketing emails from us by following the unsubscribe instructions included in each email.
  •  Compliance: To comply with legal obligations and to protect our rights and interests.

3. The collection of sensitive information

I understand that some information is sensitive, especially when it comes to your health. Thank you for trusting me to keep this information confidential.  I take extreme care in my efforts to collect sensitive information in a secure way. This information is needed so that we can provide the services you have requested in the most appropriate manner. 
Sensitive information may include DOB, medical history, current and/or previous medications or disease diagnosis, family history, previous pathology testing or investigation results, allergies, diet and lifestyle history. 
How sensitive information is collected:
  • Through an intake form via Simple Clinic when you book your initial appointment. I also use Simple Clinic for treatment notes during our consultations, in which all patient notes are stored securely.   • in a secure Zoom consultation. 
  • in a secure payment method (i.e. Square). 

4. Collection of Information from Minors:

Please note that all information collected from children under the age of 18 is classifed as sensitive information. This information may be collected under the following circumstances:
  • In the presence of their parents.
  • With their parent/guardian present.
All information collected from minors is securely stored in accordance with this privacy policy. 

5. Sharing Your Information

We do not sell, trade, or otherwise transfer your Personal Information to outside parties except as described in this Privacy Policy:
  • Service Providers: We may share your information with third-party service providers who perform services on our behalf, such as payment processing, data analysis, email delivery, hosting services, customer service, and marketing assistance. This includes Simple Clinic, Vital.ly, Ariya Health, Natural Chemist, and Square. 
  • Legal Requirements: We may disclose your information if required to do so by law or in response to valid requests by public authorities (e.g., a court or a government agency).
Other instances where I may disclose your information:
  • to provide you with the appropriate services or the services you have requested.
  • to refer you to other service providers if necessary or at your request.
  • Where disclosure is necessary to carry out your instructions, such as corresponding with someone on your behalf, requesting pathology, ordering supplements. 
  • Where i use support services to assist me in my business.
  • To engage in professional supervision or mentoring - here, any information I share is de-identified to preserve client confidentiality. 
You consent to me sharing relevant information when required with:
  • People you authorise me to correspond with, as reasonably required to carry out your instructions
  • My employees/subcontractors.
  • Third party providers who assist with:
    • accounting
    • administration
    • auditing
    • business consulting
    • email marketing
    • legal or financial advice
    • professional supervision
    • technological services
  • I will also need to disclose your information if required by law in response to a subpoena, discovery request or court order, in compliance with mandatory reporting obligations or in circumstances permitted by the Privacy Act - for example, where I have reasonable grounds to suggedst that someone is engaging in unlawful activity, or misconduct of a serious nature, that relates to my work with you. 
  • I may also disclose information to an appropriate authority if I have serious concerns about your health, safety and wellbeing.
